Privacy notice
How Brookhaven Family Dental handles your personal data, in line with UK GDPR and the Data Protection Act 2018.
Who we are
Brookhaven Family Dental is the controller of the personal data you give us through this website, by phone or by email (ICO registration ZX000000). You can contact us at hello@brookhavenfamilydental.co.uk or on 01632 960600.
What we collect, and why
When you enquire, we collect your name, contact details and a short note of how we can help, so that we can reply and, if you wish, book you in. Our lawful basis is taking steps at your request before providing care (UK GDPR Article 6(1)(b)). Please keep health details in the form brief; it is better to discuss those with us directly.
If you become a patient, we hold your dental records to provide your care. Health data is special category data, and we process it to provide health care (Article 9(2)(h)), under a duty of confidentiality.
This website
We measure visits without cookies and without storing anything on your device. A map only loads from Google if you choose to open it.
Our chat assistant
If you use the chat assistant, your messages are processed by a third-party AI service to generate a reply, which may involve processing outside the UK under the safeguards the UK GDPR requires. Please do not share health details in the chat. If you ask us to follow up, your contact details are stored with the rest of your enquiry.
Who we share it with
We never sell your data. Website enquiries are handled for us by our website provider under a written data processing agreement. Within your care we may share information with other health professionals, with the NHS where you receive NHS treatment, or where the law requires.
How long we keep it
Enquiries that do not lead to an appointment are deleted within 12 months. Dental records are kept for the periods set out in the NHS records management code of practice.
Your rights
You can ask to see, correct or delete your data, to restrict or object to how we use it, and to receive a copy. Contact us and we will respond within one month. If you are unhappy with how we have handled your data, you can complain to the Information Commissioner’s Office at ico.org.uk.
This is a general template and should be reviewed by the practice (and, where appropriate, a legal adviser) before publication.
